Your insurance policy will provide information regarding your benefits and
what is covered under your insurance plan. We encourage our patients to become familiar with their policies and contact
your insurance company if you have questions about pre-certification or referrals or authorizations. It is up to you
as the patient to obtain any referrals necessary prior to arriving for your appointment. If you do not obtain your referral
we will expect payment in full at the time of the visit. We participate with the majority of the major insurance companies;
you can verify this by calling your carrier prior to your visit or prior to scheduling your appointment.
Our office policy is to collect all co-payments at the time of your visit;
if you are not prepared to pay when you arrive we will reschedule the appointment for a later date. Also, keep in mind
that our patients are repsonisble for paying for their services within a reasonable amount of time, regardless of the status
of the claim. We will bill you for services after your insurance company either pays or denies the service. Any
deductibles applied to our fee or non-covered services will be your responsibility. Our physicians will only provide
services to you that they feel is necessary for diagnostic purposes and medical treatment for your condition. This does
not necessarily always meet insurance criteria of a "covered service". However, we will confirm coverage in advance
and inform you of any services that may not be covered under your plan. If the service is not covered, payment will
be required in full at the time of your visit unless previous arrangements have been made with our Billing Office. We
file all claims on your behalf to your insurance company.
Self-pay patients
If you do not have medical health insurance or are receiving medical care that
is not covered under your insurance plan, you will be considered a self-pay patient. Self-pay patients are expected
to pay for their visit in full on the same day of your visit, unless previous arrangements are made with our Billing Office.
