In order to enhance the level of service that we provide you, we ask that you
complete your patient demographics, history forms in advance through a new online service called eClinical Works patient portal. Using
the patient portal will save you time at your appointment and ensure that your patient information is accurate. You
will also be able to access lab results, request future appointments, prescription refills, and view billing statements.
In order to access the patient portal you must first be assigned a username
and password. You can get this information from our staff when making your appointment. Once you have your username
and password click on the link below and it will automatically connect you to the patient portal.
